Medical Expense Plans: These health insurance plans pay diagnosis and treatment costs. The payments can be made to you directly, or to your medical provider.

Reimbursement Insurance Plans: If you are involved in a full freedom-of-choice plan, then you can have the freedom to choose any hospital or doctor that you want. However, you must pay a deductible before the insurance company pays its benefits. The higher the deductible you state that you are willing to pay, then the lower the cost of the insurance.
In PPO Plans (Preferred Provider Organizations), you can only choose a doctor or hospital from the list that they provide to you (to receive full benefits). If you do not, then you will be covered for less than is stated in your insurance rates policy.
